No middle name

Question: I have had my passport for over a year and have traveled a bit but it does not have my long two-worded middle name. How can I add it?

Answer: Since you're not submitting a legal name change document, you have to submit a new application as if it's your first time.

Answer: It is not necessary for both of your passports to exactly match. What you must remember is to book tickets to match the passport you will be using to travel.

Drop one of two middle names

Question: I’m applying for passport renewal for a child under 16 and would like to drop one of the two middle names on his current passport. Will simply dropping one of the names on the DS-11 application be accepted?

Answer: Since the child will not be presenting identification documents, the only basis for his name will be the birth certificate. The passport will likely be issued in the name that's printed in the said document.

Answer: The best way to do what you need is to submit a new passport application. This way, you can simply enter your names the way you need them to appear in the passport. You need to use Form DS-11 for this.

Question: My husband is a Colombian and he’s traveling to Colombia. His Colombian passport has his middle name but his U.S. passport does not. What could happen when he comes back to the USA?

Answer: Nothing should happen. As a U.S. citizen, he needs to use his U.S. passport to exit and re-enter the United States. As long as his tickets are booked in the name that appears in his U.S. passport, he should not have issues.

Middle name misspelt on BC

Question: I just got my baby's birth certificate yesterday and didn't notice until I got home that her middle name was misspelt. We have an appointment at the passport office tomorrow and we are due to travel in two weeks. We still have to apply for a visa so we do not have time to do a change of name at the moment. Will it be a problem if I fill out the correct spelling of the middle name on the passport form.
