multiple name changes due to divorce and marriage

ID and SS Card differ
by: Anonymous

Question: I recently married in 2022. When I got married, I went to the SS office and changed my name with them first. My husband is Puerto Rican and has two last names. At the SS office, I took only his first last name. I hyphenated my maiden name with his first last name with no issues.

I then went to the DMV to change my DL. My DMV will not allow me to change my Driver's License to the name on my social security card because my husband technically has 2 lasts names, which are both present on our marriage certificate. They say I must take both of his last names. I do not want to have three last names so have been unable to change my license.

Am I able to get my passport without my DL having my new name? Despite my SS card having my new name?

Answer: Yes, you still can. If you would like to have your hyphenated maiden name on your passport, you need to submit a copy of your SS card and one other secondary ID bearing said hyphenated name.

DL and passport
by: Doreen

Question: Hello. My maiden and husband's last name was hyphenated after I got married.(ex: Doreen M. Smith-Scott) However, I was divorced 15 years ago and never went back to my maiden last name afterwards, as it was never requested during the divorce.

However, I was just granted a name change to return to my maiden last name. Currently, my DL is still in both my hyphenated maiden and married name. I had applied for my passport before being granted my name change which, was luckily is in my maiden name only (Doreen M. Smith). I plan to travel to Mexico soon. Will I have a problem leaving the country because I haven't changed my DL and ID card to only my maiden name yet? Does my passport and ID's have to match exactly to travel out of the country? My airline tickets to Mexico are booked in my maiden name, just like it appears on my passport. Thanks.

Answer: Hello, Doreen. As long as your passport and tickets match, there shouldn't be any issues. Have a safe and fun trip!

SSN and Naturalization Cert do not match passport or DL
by: Anonymous

Question: The last name on my SSN card, naturalization certificate, and birth certificate do not match the last name on my passport and DL. I have never gotten a name change so I do not have any documents to prove a name change - it is just how it was done years ago.

Will this cause an issue? Driver's license name cannot be updated since passport does not match birth cert and SSN, etc. My passport has expired - what do I need to do to have the passport agency update my last name to be consistent with other documents?

Answer: You may submit a new application in the name that matches your birth certificate. Since your primary ID (license) bears a different name, you must submit a copy of your SS card and one other secondary proof of identity with your application.

Can I use my SS Card as proof of name change?
by: Julie

Question: My original passport was issued in 1998 before I was married. I went to get a new one and they stated I needed my Marriage Certificate. I don't have that anywhere. At some point, I changed my last name on my SS Card, and I have that. My DL also has my married name. Can I use those 2 items as proof of name change in lieu of a marriage certificate? Also, should I be applying for a new passport as if one was never previously issued?

Answer: You cannot use your SS card as proof of legal name change. Since you changed your name via marriage, the only acceptable proof of legal name change is your marriage certificate.

Having said that, you do not need to submit proof of legal name change for a new application which you should be submitting. Since your most recently-issued passport expired over 5 years ago, you need to apply as if it's your first passport. The new passport should be issued in your married name since your primary ID (driver's license) also bears that name.

When I changed my name through SS did my passport with another name become invalid?
by: Anonymous

Question: I got married in 2021, and changed my name through social security and all. We are mid-divorce now but it’s not finalized. I have a social security card in my married name. I have a passport in my maiden name.

I am traveling internationally in April next year. My divorce will not be final until February. I requested a name change through the court, however, I know I will not get enough time to change my name through social security and get a whole entire passport by April.

My question is when I changed my name through social security, did my passport with my maiden last name become invalid? Can it not be used any longer or is it still valid? My fear is going through customs and them telling me there’s an error with my passport or it being blacklisted because my name has legally been changed.

Answer: You may continue to legally use your passport even after the divorce is finalized. Just make sure to book your airline tickets in the name that appears in your passport to avoid issues with TSA.

ID doesnt match the name I provide in the application

Question: Hi, I submitted my passport application a while back and I just received a letter saying that the name I provide doesn't match my ID, it states "please submit updated identification in the name (my name) (last name)". Answer: You should submit an ID matching the name you entered in the application form. If you do not have any, please contact the NPIC at 1-877-487-2778 to find out what your options are.

SSN Name Mismatch with my Passport Name
by: Anonymous

Question: Hi, I am an international student and my last name has 24 characters. Married Name in Passport, Social Security Office won't honour the Passport to change it in their system
by: Anonymous

Question: I had gotten my passport before I married. I married overseas, and the marriage certificate for that is in that country's language and in that country. I submitted a copy of that with a passport application so the current passport is correct. I did that overseas.

The problem now is that Social Security is refusing to accept the corrected passport as proof of my NAME and won't change it in their system.
That makes me get a "name mismatch" on everything I apply for. I'm not sure if I'll be able to get a driver's license (I'm back home now) but I was able to get library cards replaced. Oh and what happened to my original marriage certificate, my bag has gotten stolen and my papers stolen so many times while I'm overseas that I've lost count. The only replacement is to send to that country through its Consular Services at its Consulates here, for another "original."

Why is Social Security being so strict about this? When I explained this same thing to the offices in Massachusetts and Connecticut they were willing to work with me, and told me to bring in the passport in person. Out here in California now, whole different ballgame. And they're insisting this is not "document abuse" when it's the very definition of document abuse. I went into the office in person with the passport and the person behind the bulletproof glass said no. And that this wasn't "forcing" me back into my maiden name or being discriminating (they didn't do this to my mother's generation!). So now, what, I sit up and wait four months for a copy of my passport records from the Department of State since the copy of the certificate will be THERE. FOUR MONTHS!?!?

Answer: A passport, aside from being a travel document, attests to your citizenship and identity. It is not, however, proof of legal name change. Government offices will only accept the actual document that you signed to change your name which in your case is your marriage certificate. This is not document abuse, certain documents serve certain purposes only.

Name change?
by: Anon88

Question: I renewed my passport and it has the following example name

Carlos Jesus Lopez Guerrero

But my social security card only has Carlos Jesus Lopez

Can I simply resend my passport to have them drop Guerrero name in order for it to match properly to social? Reason for this is California's new id requires all names to match. And do I need any special documents to correct passport names or is the passport enough?

Answer: If you're not going through a legal name change, you need to submit a new passport application and send copies of IDs that do not have Guerrero in them. Your SS card plus another government-issued ID should suffice.

If you submit your current passport for a renewal without a legal name change document, the new one will just be printed in the same name as the old one.

Question: I will be getting married soon and want to do a name change to my legal documents (license, passport, etc). As of right now my passport and license match in name but are missing my second last name (shown on my SSN). Do I have to fix my passport and license to match my SSN before doing a name change with my new last name? Or am I able to change my license/passport to match my SSN with the addition of my new last name?

Answer: Your passport will be issued in the name that is supported by the name change document you submit with your application. If that name includes your second last name, the passport will likely have that name as well. As for your license, please contact the DMV.

The last name on my social security is different from my other documents.
by: Caiden

Question: I am wanting to get a passport so I can study abroad. I am told by my parents that I will not be able to do so because my last name is different on my SSN. I have tried to get a name change done, but they want my original birth certificate, which was destroyed in Hurricane Katrina.

My current copy of my birth certificate is not enough for that, so I will most likely have to wait until I'm 18. Is it still possible for me to get a passport in this situation?

Answer: You need a state-issued birth certificate in order to apply for a passport. We recommend that you file a request at the city/state vital records office. Once you have your long-form birth certificate, you may change your name. And then you may proceed and apply for a passport.

Applying for a passport with a recent Name change
by: Brendan pg

Question: I’ve just had my name changed, I haven’t updated my driver's license or social security card name.

Due to wait times, I need to apply for my passport soon. Would it be an issue if I applied for my passport with my old last name? Passport name doesn’t match Drivers license

I got divorced a few years ago and had my name changed back to my maiden name. ALL my documents have been changed except for my passport.

Passport middle name different from everything else
by: Anonymous

Question: When I got married, I got my new passport printed with my maiden middle name and married last name. Then, when I got everything else done, I changed my mind and put my maiden last name as a new middle name.

Now every identification (social security, driver's license, military ID, mortgages, etc) has a different middle name than what is on my passport. However, when trying to change my name on my passport, they won’t let me without a court order. Since the name on my passport was never my name, there is no court order I can get to change it. Will I just always have a different name on my passport than on everything else? Is there anything that can be done?

Answer: You can submit a new application as if it's your first time. The passport will be issued in the name you enter in the form as long as it is supported by the primary ID (driver's license) you present with it.

This was NEVER done formally but all my adult documentation including my social security are in the name Jones. I'm 40 years old now. Never been a problem but now I need a passport.

What should I do?

You need to submit your application and requirements at an acceptance facility. The general requirement is only 1 ID but if you can provide more, send photocopies of IDs that have Jones on it.

Can this be corrected when she renews her passport? What type of supporting document she needs to provide? Thanks.

Answer: Her passport will be renewed in the name printed on the old one unless she sends proof of legal name change. Since this is not a name change per se but just a removal of the extra last name, she needs to submit a new application in person instead of a renewal by mail.

The passport will be issued in the name she enters in Form DS-11 since it will be supported by her primary ID (driver's license). She needs to submit the application in person at an acceptance facility.

Please note that standard service takes up to 18 weeks and expedited service up to 12 weeks. If you have any plans to travel in the next few weeks, we recommend waiting to apply for the new passport until after your trip. Just make sure she books airline tickets in the name that's on her passport.

Name on passport reversed
by: Anonymous

Question: My name on SS, DL and everything else is the correct one but when I became naturalized, my first and middle names were switched. Consequently, my passport has that order. How can I change the name on my passport? Thanks.

Answer: You need to submit a new application as if it's your first time. Complete the application form with the correct order of your name and submit a photocopy of your driver's license bearing said name in correct order. We recommend also including a signed request for the passport on your passport to be printed in the order you entered in the application form as supported by your ID.

Applying for name change on DL with pending passport application on old name
by: Anonymous

Question: I just got married, but don’t have my marriage certificate yet. I also will be traveling internationally and need a passport but never had one before.

Can I apply for a passport with my old last name (as on my IDs) AND at the same time apply for a new SS and DL with the new name once the marriage certificate arrives? Or will this potentially delay the passport approval?

Answer: Your name change with SS and DMV will not cause any delays with your passport application.

However, applying for your passport in your maiden name could potentially pose a problem when you apply for a passport name change in the future. Your marriage date precedes your application date so you are expected to apply for the passport in your current legal name.

We recommend waiting until you receive your marriage certificate to apply for a name change on your IDs and then apply for your passport. Also, we strongly recommend not making any final travel arrangements until you receive your passport. Processing of passports take up to 18 weeks now and signicant delays can be expected.

MIDDLE NAME
by: Anonymous

Question: My birth certificate has the middle name spelled Cesilia, my drivers license and SS card are spelled Cecilia. I am wondering if I will have any issues while applying for a passport.

Answer: Since the two names are common derivatives of each other, you should not have issues applying for a passport.

My passport has my maiden name and doesn’t match the name on the COVID card (married name). Will this be an issue to flight internationally?

Answer: It should not be an issue but we recommend bringing a copy of your marriage certificate to present should questions about the name mismatch arise.

Real ID and Passport Name Match Req'd
by: Anonymous from OH

Question: Getting ready to renew passport. The current/expired one does not list my middle name. Recently acquired Real ID lists full mid name (DMV requirement; not my request). Should I list my middle name on the passport renewal application to be consistent with Real ID? If so, do I need anything special to add this (since technically it's a 'change' from my existing PP name?)

Answer: While ideal, it is not absolutely necessary for your passport and real ID to match. If you do want to have your middle name on your passport, you need to submit a new application instead of a renewal. This means you need to apply in person and pay the execution fee on top of the application fee.

Please set an appointment and submit the following at a local acceptance facility:

1. Form DS-11
2. Citizenship evidence
3. Government-issued ID bearing the name entered in DS-11
4. Passport photo taken in the last 6 months
5. Payment for fees

Question: Hello. I am trying to get a DL and they won't allow me because on my SS card, I have 2 last names and my passport only has 1. Should I change my passport or my ss card?

Also, my Birth Certificate (from a foreign country) has a middle name. How do I get my middle name added to my passport? I would like all my IDs to be consistent to avoid any issues.

Answer: You may submit a new passport application and enter your middle name in the form. This type of application needs to be submitted in person at an acceptance facility and you also need to pay the $35 execution fee.

Please note that processing currently takes 10 to 12 weeks and there may be some delays. If you have travel plans, it is better to postpone the application until after your trip.

Question: Hello, I'm wondering if it's okay to go ahead and change my name in my passport first before my SS card as my passport is expiring soon. Also, because of my husband's culture, wives take on the husband's first and last name while dropping their own middle and maiden names to accommodate that. Will that be a problem since I'm changing both my middle and last name? Thank you.

Covid vaccine card
by: Anonymous

Question: I recently received my Covid vaccines. The name on the card matches my real ID but not my passport. Passport has my first name as a middle initial and middle name as my first(I have always used my middle name as my first. Will that cause issues when I fly internationally?

Answer: Since the vaccine cards have not been established as a requirement for all international travel, the mismatch may not be an issue. Just show proof of identity.

We also recommend that you contact the airline you are flying with to double-check their requirements and to find out if the name mismatch will be an issue with them.

Question: My passport has my hyphenated name and even though my social security card has my maiden name and not my hyphenated name, the new airline I was just hired at is saying I have to change my passport back to just my maiden name or I will have trouble flying internationally.

My last airline I had no problems with this! Ex: My SS card Jane Doe, my passport: Jane Doe-Smith. Why would I have to change my passport to match my SS card to fly?

Answer: While it is ideal, it is not necessary for your passport to match your SS card in order to travel internationally. Having said that, it may be company policy to have all your IDs match. You need to clarify this is human resources.

Also, contacting TSA may get you more information on identification and international travel.

Question: I got married and I changed my name on my SS card but I have yet to get it changed on my DL. The DMV is now packed, by appointments only, and I don't have much time to apply for a new DL to catch an international flight. This is my first passport, will I have issues getting a passport if my DL and birth certificate are in my maiden name and my social has my married name?

Answer: Driver's licenses are accepted as primary identification documents whereas SS cards are only secondary. This means that if you submit your SS card as proof of identity, you will have to submit another secondary ID bearing your new legal name. You may also have to submit the original or a certified copy of your marriage certificate to further support your name.

Different First Name
by: Anonymous

Question: I recently went to the DMV to renew my drivers license and upgrade to the new REAL ID but was unable to. My SS card and birth certificate list my first name as Toni but I have all other documents/ID's/DL#/passport which lists Antonia as my first name. I was told from my grandparents (who raised me) that was my name (Toni is short for Antonia)and have always gone by my longer name in school, work, etc.

The problem is that I cannot get a REAL ID because it does not match my birth certificate and SS#. They recommended that I get a legal name change but I am wondering if I should get a new passport with Toni vs Antonia. Can I do this even though my DL# shows Antonia as my first name?

Answer: It is ideal all your names match across all legal documents including your passport. You can either follow what's on your birth certificate and change your license OR you can have your name legally changed and have it on your birth certificate, SS card, DL and passport thereafter.

However I am changing my name on my social security card, Driver’s License ID, and every other form of identification. Will my passport still work to travel internationally even if it has my maiden name on it while everything else (specifically my social security card) has my married name? Name Change in Expiring Passport
by: Mali

Question: Hello! I got married end of last year and my passport is expiring April 2021. I have not yet started my name change process for Social Security, driver's licence, or my passport due to Covid-19.

1) Can I just change my name for my passport for now to match my marriage certificate before it expires and update Social Security & driver's license with my name change later without any issues?

2) Due to Covid-19 impacts, if I submit my documents through mail to renew my passport, what is the estimated time frame to get my renewed passport with my updated name change? How would I check the status of my passport renewal if I send it in through mail?

3) Will I get all of my submitted documents back, such as my marriage certificate?

Thanks!

Answer: 1. Yes, you can.

2. Renewals with name change can only be done by mail now. There are no set timelines for processing due to limited passport services but those who have submitted applications have been told it takes 3 to 6 months. You may check the application status online here: http://uspsg.net/status/

3. The supporting documents you submit will be returned when the process is completed. However, delays are still to be expect in their return.

Rejected passport application
by: Kat

Question: Hey! I am very confused. Please help me. I went to the social security office with my passport to get a new social security card because I lost my original one. The social security office told me my name is different on my passport vs the name on my social security card so he can’t give me a social security card until I correct my name on my passport. He said I don’t need to pay for a name change that I should just apply for a new passport and write my correct name. The passport application got rejected. I don’t know what to do at this point :(
I can never get a credit card because of this!

Answer: Hi, Kat. You were informed incorrectly. You need to pay for the passport application. If you did not include a check, it might have been the reason why the application was rejected.

If you did include a check, the rejection may have been because of the discrepancy between your names. The name on the application has to be supported by the ID you submit with it.

If you'd like a clarification, please contact the NPIC at 1-877-487-2778.

Social security card in married name but ID in maiden
by: Anonymous

Question: I was married in July 2019. Finally changed my name officially with social security to my married name after having my passport stolen in January 2020 so that when I got my new passport, I would get it in my new name. Then COVID19 happened. I have not been able to go to the DMV to get a new drivers license and do not know when I will be able to. So I now have a social security card with my new name- and an ID that has my old name. How do I go about getting a new passport in my married name without a drivers license showing my new married name? Very frustrated.

Answer: The social security card is considered secondary ID. If you can present another valid photo ID bearing your married name, you can still get your passport issued in that name.

My married last name is hyphenated and wouldn't fit on my state's license, and the DMV told me to keep it in my maiden name.

Breaking up the name change process
by: Anonymous

Question: I'm changing my name because I was recently married. I will travel internationally soon enough to where I don't have time to update SS, DL, and Passport all before I leave, and then I will shortly thereafter travel domestically. Can I change my SS card now and then do the DL before the domestic trip and the Passport once I'm back for good? Does domestic or international travel require two of these three documents to match? As long as I book the international travel in the same name as my passport and the domestic travel in the same name as my DL (regardless of if they agree or not), will I fly without issue?

Answer: Looks like you have it all figured out. As long as your tickets match the name on your passport/ID when you travel, you should be okay.

What to do?

recently got my usa passport

Question: When I went to the DMV to get a drivers license, they said that the name on my passport doesn't match the name on my social security card.

I just took the learners permit, there was no problem checking in there with the social security and passport.

When I went to get the card, they mentioned I cannot get the license because the two forms don't match. Why?

I have 4 names on my passport and birth certificate.

My social security card only has 3 of the names.

What do I need to do?
What are my options?

Answer: Since the issue is with your driver's license, you need to contact the DMV for advice.

Birth Certificate/Passport name different than Social and Drivers license

Question: Hello. When I became a citizen of the United States 11 years ago, my passport name was spelled how it is spelled on my birth certificate. Thanks in advance.

Answer: If you can change the name on your IDs to match your birth certificate and passport, please do so. But if you want it the other way around, it is best that you get a court-ordered name change and amend your birth certificate and apply for a passport name change.

Name change to match on all her ID cards
by: Anonymous

Question: When my mother entered the country in the mid-1960s, she received her SS card with her full name matching what's on the green (alien) card. When she applied for naturalization in the early 1970s, her first (maiden family) name was dropped on her citizenship papers. The reduced name has appeared on her passport ever since but her SS card and subsequent Medicare blue/red card retained the original full name.

What is the best and easiest way to add back the full name on her passport as the missing first name different names between ID cards has caused some ID confirmation issues. We would prefer to add back than to drop the first name on her SS and Medicare card as all her medical records contain her full name. Through search of National Archive, I have a copy of my mother's original naturalization application showing the name dropped, in an official CD copy. This is the only official document that records my mother's name change history. Unfortunately we have no other certified documents. Please advise. Thanks.

Answer: Your mother needs to submit a new application. The passport will be issued in the name entered in the form as long as it is supported by the ID/s she presents with the application.

Passport name is different from other documentation
by: Anonymous

Question: I need to renew my passport since it is expiring soon. I also would like to request a data change since my current passport uses my "aka" name that I obtained while being naturalized as a U.S. citizen.

Instead, I want my passport to match my driver's license and my SS name (legal name, not the aka name). It got really confusing after all this time knowing that my passport was the only document with a slightly different identity, so I thought this would be a good time to get that changed while renewing it at the same time. It's not an official name change, but rather a "correction" since both names are technically on my Certificate of Citizenship, so I believe I need to submit the DS-82 and the DS-5504. Am I right? Would I also need to submit my original certificate of citizenship as a proof of my legal name?

Answer: Since you cannot present proof of legal name change, you need to apply for a new passport in person. Please prepare the following and submit at an acceptance facility:

1. Form DS-11
2. Naturalization certificate plus photocopy
3. Proof of ID plus photocopy
4. One new passport photo
5. Payment for fees

Legal Name Change
by: Anonymous

Question: I am going on a last minute trip abroad with my children. I have been married for 7 years. I changed my last name to my married name (when we bought our house 5 years ago) on my license at the DMV, so I go by my married name on everything (all of our bills, any information I need to put my name down for our children, our health ins., EVERYTHING etc.), but LEGALLY I never changed my last name on my social security card. I am renewing my passport (which has my LEGAL name, not married name). Will this be an issue traveling, esp. with my children? Which brings me to my next question if it is an issue, in order to change my name on social security card, I need to bring a NOT expired drivers license, and passport.. both of these are expired, and NOTHING else of mine has my maiden name (as I stated I changed my name at the DMV, so I go by my married name for everything). What is the best plan of action in my situation?

Answer: Your legal name is your current married name even if you did not change it via Social Security. In light of the recent changes in the ID system, it is highly recommended that all your documents match -- especially your license, SS card and passport. Your best course of action is to apply for a new passport in your married name.

Different last name in passport
by: Geronimouh

Question: My last name was changed about 20 years ago when my mother was married again. I have the old last name in my social security card but i have the new name in my U.S. passport. I went to social security office to get a new social security card with my my updated name. Middle name
by: Anonymous

Question: I recently renewed my NY driver's license. When I got married years ago I took only the middle initial of my maiden name since I was never given a middle name at birth. I was advised at DMV with the newest license I had to state my full maiden name as my middle or drop it completely. Since my SS and passport was changed with the initial, I decided to enter my full maiden on my license.

Moving forward, will it be an issue if my license has a full middle name but my social and passport only show a letter as a middle? Both my first and last name are the same on all documents.

Answer: It should not be an issue. It will be unlikely that you will need to present your license and passport when you travel.

Question: I changed my last name as I got married recently and have my new social security card. I haven’t changed my name with the DMV yet. I need to apply for a passport since we are traveling in the next two months . Can I take my new SS card and my ID with my maiden name and obtain one?

Answer: If you want your passport to have your married name, you need to present a primary ID (driver's license) or at least 2 secondary IDs (SS card plus another ID) with your married name on it. We recommend changing the name with DMV first to make the passport application process easier.

New passports are issued in the name entered in the application form as long as that name is supported by either the applicant's evidence of U.S. citizenship or proof of identity.

Name changed during green card interview
by: Maani

Question: I wanted to change my last name as it was too long and got an idea to ask my officer during the green card interview. They allowed to change my last name. My green card came with the new last name and I was able to change my SSN card. But when it came to my banks and real estate license, I encountered a problem as they all wanted a prove of my name change(court letter). I literally don't have anything other than my green card and SSN. I could convince some and some would not accept.

Now for the first time, I'm going on a trip outside of the US with my Iranian passport. The country of destination doesn't need visa for green card holders but needs passport anyways. My passport last name and other US documents wont match now! So, is there gonna be any problem going and coming back?

Answer: There may be some questions that will arise but as long as your green card and passport are valid, you can still re-enter the United States. We recommend that you contact the U.S. Customs and Border Protection office before going on your trip to see if you need any other special documents to facilitate re-entry.

Answer: Yes, you may still apply for a passport. Your passport will be issued in the name that fill the application in. Since you cannot submit primary proof of identity (driver's license), you may submit a combination of secondary proof of identity. This can be your Social Security Card and any other ID that shows your current legal name.

Question about SS
by: Anonymous

Question: I currently have a passport card but I want to fix the name to match my social. My first name is pronounced the same but my last name the first two letters are in different spots. I doubt that Ii need a court order. Can I just send in documents with the name how it is on my social?

Answer: The names on your passport and social security card do not need to match. But if you want to change the name on your passport, you will need a court order as evidence of legal name change.

Another option is to submit at least 3 public records that prove exclusive use of the name for at least 5 years. More often than not, applicants who go this route are asked to submit more documents which delays the process. We recommend that you provide primary evidence of legal name change to avoid issues.

Name on SS card doesn't match name on passport or DL
by: Anonymous

Question: How do I go about changing my name on my passport and DL to match that on my SS card?

Answer: You need to change the name on your driver's license at the local DMV office. To change the name on your passport, you need to submit the following:

1. Form DS-82
2. Current passport
3. Evidence of legal name change
4. Payment for fees

If it has been less than a year since your passport was issued, please use Form DS-5504 instead of DS-82. There will also be no fee if you use Form DS-5504 unless you want the process expedited.

by: Anonymous

Question: I have a passport with my previous married name on it and I am now re-married. Can I still use that passport for my upcoming trip to Mexico? My driver's license and Social Security number are under my new married name.

Answer: Yes, you may travel to Mexico with your current passport as long as it is valid and you book your tickets in the same name that appears in it.

Maiden name on my social but not ID
by: Carla

Question: I am planning to apply for my first passport. I am married and changed my name with the DMV but never changed my last name with the SS office. I intend on using my married name on my passport application, would that cause any delays?

Answer: It should not cause any problem if you present and submit a copy of your driver's license. Since you are applying for your first passport, we also recommend that you include the original or a certified copy of your marriage certificate as proof of legal name change. All your documents will be returned to you.

Process of name change
by: Anonymous

Question: I booked my flight with my married name/name on my passport but have recently changed my name on my SS card and drivers license (due to divorce) back to my maiden name. Is my passport with my married last name still valid?

Answer: TSA's Secure Flight Program requires that the names on your airline tickets and passport match. You should have no problems boarding your flight.

However, in case you'll be asked to present additional identification, it may be prudent to bring a copy of your divorce decree to establish the link between the name on your passport and the name on your IDs.

What is considered acceptable documentation?
by: Anonymous

Question: My passport has my married name and I am recently divorced. My divorce decree did not include a name change, but I have recently updated my DL and SSN to reflect my maiden name which is in my birth certificate. I want to change the name on my passport back to my maiden name.
I read that I can try to show 5 years of documentation showing that I have used my maiden name, what is acceptable documentation?

Answer: Since you cannot use your divorce decree as evidence of legal name change, you need to submit at least 3 public records showing your date and place of birth that show exclusive use of your assumed name for at least 5 years.

Please note that you may be asked to send more documents which could delay the process. We recommend you get a court order for your name change.

Flight is tomorrow
by: Anonymous

Question: My driver's license has my first, middle and last name and booked my flight with using it too. My passport does not have my middle name on it. Could this cause me a problem?

Answer: TSA may provide some leniency with regard to middle names. We recommend having your driver's license handy in case you're asked to provide additional identification. Also, please contact the airline you are flying with for advice on the matter.

Marriage certificate does not match
by: Anonymous

Question: I'm trying to obtain a passport but just realized that my marriage certificate has the last name of my first marriage on it. Can I still receive a passport or can I still apply for one?

Answer: If you are applying for a passport for the first time and your identification document has your married name on it, your marriage certificate should not be necessary. However, some agents do request it and, therefore, we recommend that married applicants send them.

Since your marriage certificate does not have your current married name on it, you should not send it unless you were issued a passport in the past. Your application may be processed. If a name change document is requested and it has not been more than five years since the change occurred, you will need to obtain a court ordered name change.

If you were issued a passport in the past, a name change will be required with your application form.

Name change prior to 9/11
by: Anonymous

Question: Hello. Prior to 9/11 I was able to add my step-father's last name to all documents. I never went to court to officially have it changed. My social security card has both last names (not hyphenated), my license has my step father's last name as my last name and my birth last name as middle name along with my middle name, my taxes have been filed under both last names. In applying for a passport would I apply with both last names or just the name on my birth certificate?

Answer: You should enter your current legal name on the passport application. Since you did not petition for a court ordered name change, you must submit multiple documents that prove exclusive use of your name for a period of more than 5 years.

Change Passport to SSN name
by: Anonymous

Question: My passport slightly misspells my name. I am in the process of renewing it and requesting a correction, but do not have my birth certificate. Is my Social Security Card sufficient evidence of my correct name?

Answer: Your Social Security card is not acceptable evidence. Please submit your original or a certified copy of your birth certificate if the passport is in your birth name or legal name change document if it has been changed. We recommend you also include a brief letter explaining the error with your passport renewal application.

Renewal DL Passport & SS card differ
by: Confused

Question: My passport has: first name, middle initial, an initial for my maiden name and full married last name. My SS card has all four names spelled out and evidently considers the forth name (married name) as my legal last name. My DL has all four names spelled out but it is hyphenated between maiden and married name (evidently this considers the last name as maiden-married). Can I just renew the passport card as it is, or do I need to apply for a new one matching my drivers license, or is none of this even possible since the DL is hyphenated and the SS is not? I'm so confused.

Answer: You can renew your passport card as it is. The name in your passport does not have to match exactly the name on your driver's license or SS card.

What about dashes in first name?
by: Anonymous

Question: My birth certificate and social security both have a dash in my first name (Anna-Belle), but my passport recognizes it as first and middle (Anna Belle). My driver's license has it together without that dash (Annabelle). Should I get a name change (court order) to fix it all or can I simply change the passport?

I am worried if this discrepancy will hurt me later on in life.

Answer: If your birth certificate lists Anna-Belle as your first name and that is how you want it to be listed, then it should not be necessary to obtain a court order. You can apply for a correction to your passport without any additional cost. Mail a completed form DS-5504, passport, birth certificate and a brief letter of explanation to the address on the form. There is no fee unless you request expedited service.

by: Anonymous
