Name change

Question: My marriage license does not have my name change on it. What do I use as the date of my name change and the city and state? When I changed it on my driving license or when it changed for my social security card?

Answer: Please enter the date and place that your wedding took place. Also, please send your marriage certificate; a marriage license is not acceptable proof of legal name change.

Do they need every previous marriage certificate ?

Question: My passport was under my second husband's name. He passed away and I remarried, hyphenating my last name. When I went to get a new passport, they said I had to have all three marriage certificates, even though my passport was under my second married name.

Do I really have to get the certificate from my first marriage, if my passport was under my name from my second marriage?

Answer: The Department of State may be asking for all these documents to establish a connection between the name on your citizenship evidence (birth certificate) and the name you are electing to have on your passport. Please contact the NPIC at 1-877-487-2778 for more information.

Question: My marriage certificate doesn't have my name change. Do I still submit it as name change documentation? Would that still be accepted as documentation needed? I only have my social security card.

Answer: Yes, your marriage certificate will be accepted as a valid name change document on the premise that you are taking your spouse's last name through the marriage.

Changed my last name but they didn't return my marriage certificate

Question: In order to change my last name I had to mail the official copy of my marriage certificate along with with old passport. They were supposed to return my marriage certificate with the new one but they didn't. Are they mailed separately?

Answer: Yes, the marriage certificate and new passport are sent in separate mailings. You should receive the document in the mail soon. If it does not arrive in the next few days, please call customer service at 1-877-487-2778.
