Question: My father applied for my U.S. passport abroad when I was young.

The country that my 1st passport has to have 4 names
My name father's name, grandfather's name and family name.

Basically, my fathers name is the only different name or we all have the same name.

On my SSN, it's 3 names, on my TWIC card it's 3 names, on my merchant mariners, it's 3 names.

Would I be able to drop one of my middle names without a court name change?

My merchant mariner and TWIC are from Homeland and coast guard approved.

With the NJ Real ID requirement coming, I am concerned that my passport (and TSA) name differs from almost all of my other IDs. I am a married woman using my husband's last name. I was born "Linda Sharon Hopkins". I married "John Smith" 35 years ago.

My current and most recent previous passports have "Linda Sharon" as my Given Name(s) and "Smith" as my Last Name. However, my DL, SS card, bank accounts, credit cards, and other residential proofs read "Linda H Smith" (the NJDMV advised me to change my SS card from Linda S Smith to Linda H Smith years ago).

I do have an old passport that reads "Linda Hopkins Smith" so I'm not sure how or why the middle name changed from my maiden to my given middle name and my maiden name disappeared altogether.

Am I going to have a problem with getting a REAL ID? Should I change my passport to Linda H Smith to align it with everything else? If so, do I need to spell out "Hopkins" on the passport or would H suffice? Would it be a "change of name", or do I apply for a new passport? I do have my birth and marriage certificates with raised seals, but as stated previously, I have been married for 35 years so I don't know if that qualifies for a name change. Help!

My middle name at the time of birth was incorrectly typed on my birth certificate.

My CORRECT middle name should have an "i" but on my birth certificate, it was recorded as "'r" - an apostrophe and r instead of i. I'm thinking the person who was processing my application thought the i was an "'r" instead.

This resulted in my passport having this wrong middle name as well.

Now that I am an adult, this mistake is quite bothersome as my middle name is basically unpronounceable.

Answer: Since this is not a legal name change, you husband needs to submit a new application as if it's his first time to apply. He needs to present and submit a copy of a government-issued ID bearing his full middle name in order for it to be spelled out on the passport as well.

Question: I booked tickets to Europe with my middle initial (K) and then when I got my passport, it is my full middle name (Kaye) now the travel agency is saying it could be a problem and might have to re-book the flights. Answer: Some leniency is given to middle names vs middle initials. We recommend that you contact the airline regarding this issue first before re-booking or changing the name on your passport. These may not be necessary.

Question: I wish to apply for a passport but my middle name on my birth certificate is different then the name on my driver's license, my birth certificate middle name is one name and my driver's license middle name is another. My social security card says Jonton as well, can I go to DMV and show birth certificate and have middle name changed to show same as birth certificate and apply for passport, will I have to have my social security number middle name changed as well before applying for passport. The middle name that's on my birth certificate is different from the middle name that's on my driver's license and social security card. What do I need to do in order to get a passport? Can I just go to DMV and have the middle name changed by showing original birth certificate to match name on birth certificate. Will the application ask if other names been used? Since I have never applied for a passport will it be easier to go to DMV and have my middle name to show what's on original birth certificate as well as social security card?

Answer: It is not necessary to present your Social Security card when applying for a passport. You only need to enter your Social Security card number on the passport application.

You do need to submit the original or a certified copy of your birth certificate and present your government-issued photo ID and a copy of the same. If the names do not match on these two documents, you must submit either evidence of a legal name change or multiple documents showing the use of the name on your ID for a period of at least 5 years. We recommend you either change the name on your driver's license to match the name on your birth certificate or, if the name on your driver's license is your current legal name, submit a Petition for a Name Change at the county court where you reside.

Question: I wish to apply for a passport for my child, but her birth certificate shows "Anthonyk" instead of Anthony, (for her fathers middle name) from a typo error. Can we still get a passport for her as long as her father's first and last name are spelled correctly on her birth certificate?

Answer: In order to avoid possible problems, we recommend you apply to correct the birth certificate first and, then, apply for the passport. If you do not have time to do this, you can submit the birth certificate knowing that it may not be accepted.
