Form to use to renew 16 and 17-year-old children's passports.

Question: What form do I need to renew my 16-year-old and 17-year-old children's passports? Do both parents need to be present when obtaining the passport? Can I do it by mail?

Answer: Passports that were issued to minors cannot be renewed by mail. Your children must apply for new passports using Form DS-11.

All Minors Age 16 and 17 shall:

* Appear in person at the time of application;

* Have a parent or guardian present photo identification if the minor does not have identification of his or her own;

* Provide a photocopy of the same ID document that will be presented at the time of application (applicant or parent/guardian);

* Establish parental consent (You may be requested to provide written parental consent for a child age 16 or 17 if it is not implied on the application).

At least one parent needs to be present for a 16-year-old to apply. Usually, a minor age 17 can apply without the presence of a parent.

If you need to get them in less than two weeks, you will need to either submit the applications at a regional passport agency or apply through a registered passport expediting service
