Middle Name Errors

Question: I am traveling to Tulum in less than 30 days and Cancun in less than 90 days. I received my passport last year after a long drawn-out process.

I had to get a new birth certificate as well in the process, which made it a lot longer. After receiving my passport back, it only has my second middle name on it but my ID has both my middle initials. Is that going to be a problem? I’m going to bring my birth certificate just in case but I’m just making sure before I waste my time.

Answer: I recommend that you book airline tickets in the name that appears in your passport to avoid issues with TSA.

Question: I booked a plane ticket to Italy using my middle initial because that is how my passport was issued. The trip is next Sept. In the meantime i had to get a new passport because mine expired. They just sent me my new passport and now my middle name is spelled out but my plane ticket just has my middle initial. Will this be a problem either in the US or abroad? And since I did not submit a name change for my new passport, why did the passport authority issue my new passport different from the old one? Thank you.

Answer: The middle name/initial does not usually create an issue with immigration. However, I recommend that you contact the airline to check if they have any regulations that might cause a problem.

Question about some mistakes I made on my DS-11
by: Anonymous

Question: Hello!

I applied for a passport and I accidentally did not put my middle name on the DS-11. I do have my middle name on my last passport though. Do you know which name will show up on my new passport? I need to know what name I should put down for my airplane ticket.

Also, I have no idea how this happened, it was kind of a stupid mistake, but on the DS-11 I accidentally put my name on the second address line instead of any actual address. The first address line is correct though and I don't live in an apartment or anything. Will this affect anything? My passport has taken pretty long to arrive, maybe they are confused by these mistakes?

I know I should have checked my application more before sending it in, but thanks for answering!

Answer: There is no way of knowing what name will appear on your passport until it arrives. We recommend waiting until it arrives before booking your tickets.

The "error" on the mailing address may or may not be an issue. Couriers usually know how to troubleshoot these issues so let's hope your delivery is not delayed.

Answer: You may write the full name on a separate piece of paper, label it accordingly and attach it to your application.

ID and middle name
by: Anonymous

Question: My driver's license has my first and last name only. Going to renew my passport that has my middle name on it. Will this delay my application?

Answer: Since you're applying for a renewal, there will be no need to present your license or any other ID for that matter. The name difference between your license and your current passport should not be an issue.

Please mail the following to the address printed on the renew form:

1. Form DS-82
2. Current passport
3. One new passport photo
4. Payment for fees

Please note that processing times change. As of this writing, standard service takes at least 8 to 11 weeks while expedited service takes 5 to 7 weeks to complete. These processing times do not include the times it takes for the application to arrive at the processing center and for the new passport to get to your mailing address. If you have any plans of traveling soon, we strongly recommend that you wait until the new passport arrives before making any final travel arrangements.

Question: I tried to get the TSA pre-check approval but my passport only has my middle initial, my drivers license has my full middle name, they refused to accept my application. Do I have to apply for another passport with my full middle name on it?

Confirm that Given Names in US passport may include patronymic
by: Liudmila

Question: I am a US citizen and have a valid passport. In my US passport [Given Names] is LIUDMILA SEMENOVNA; that is, my first name and my "middle name" (patronymic). Actually, in all documents issued in my country of origin (birth certificate, marriage certificate, etc.), there are two separate entries: First name: LIUDMILA, Patronymic: SEMENOVNA. This made my given name in the US passport LIUDMILA SEMENOVNA.

Can you please confirm that in fact, the last name is recorded in the US passport "as-is" while given names in the passport can be recorded either as first name only OR as first name and middle name (patronymic) (LIUDMILA SEMENOVNA).

Such confirmation was requested in my country of origin to ensure my identity.

Question: My marriage Certificate reads Jodie Lee Hollis but my DL, Military ID, and SS card all read Jodie Morgan Hollis. Which is the name I submitted for my passport. (Jodie Lee Morgan was my previously married name and I chose to keep Morgan and drop Lee due to my children).

I submitted my DL and my original birth Certificate reading Jodie Lee Hundley(maiden name) with my application for a passport. I received a letter needing a marriage certificate. Is it going to be an issue that the marriage certificate reads Jodie Lee Hollis?

Answer: Since the marriage certificate also lists your spouse's last name, it should be accepted as proof of legal name change.

MIddle initial vs MIddle name
by: Anonymous

Question: Hi,

I went to renew my passport and after waiting 8 weeks, just received a letter stating that my previous passport had my middle initial, but my current passport application includes the middle name instead. All identity documents always included the middle name. Why would this stop the application process and I am asked to confirm my legal name?

Answer: Changing from middle name to middle initial actually requires a new application instead of a renewal. You should submit the documents they require or contact the NPIC at 1-877-487-2778 to find out if you can submit Form DS-11 instead.

I am just using my mother’s maiden surname but I have documents that prove that I have legal parents. They did not change it and led me to use my first name and my mother’s maiden surname only.

Middle name issues

Question: Hi, I am now having doubts if I can travel abroad because of my middle name. There's my middle name on my national ID but I don't have it on my passport. Will it affect my identity when I go abroad? Can I still use my ID together with my passport even both are not the same because of my middle name? What should I do?

Answer: I am not sure what you mean by national ID so I am assuming that you do not have a U.S. passport. As a general rule, only your tickets and passport need to match since there will likely be no need to present another ID while you travel. However, it will depend on your country's and your destination's immigration regulations.

I recommend that you contact your immigration bureau and your destination's immigration office to find out of your name mismatch between passport and ID will be an issue.

Two Last Names
by: Anonymous

Question: I will use a "First", "Middle", "Last" for my question.

I have a First, and two Last names. I was unaware and used my 1st Last name as my Middle name on several accounts I opened (bank, investment, driver license, even for my Naturalization, I don't know about my Social Security Card name, but I have just First and 2nd Last on the card, so I believe the 1st Last was used as a Middle name).

Will this cause any problems in the future? Which documents should I change, which ones should I not? Can I just use the 2nd last name for nonlegal purposes?

Answer: It may cause issues when you apply for Real ID.

Given name versus middle name
by: Anonymous

Question: I sent in my passport for renewal. My given name is John Robert, last name is Daniels.

In the renewal form, I gave my first name as "John Robert" and last name as "Daniels".

At the end of the form, it said "take it to processing center" - I mailed it nonetheless.

I was playing with the form after that and realized that when I entered "John" as first name, "Robert" as middle name and "Daniels as last name, the form said to mail it in.

I suspect I had mentioned "Robert" as the middle name in the original application and now I messed it up.

Is this a cause of concern? Will this cause any trouble with the passport application?

Answer: If you submitted Form DS-82, it should not be a problem.

Middle name on passport does not match social security or drivers license
by: Anonymous

Question: Changed name due to marriage. Dropped my middle name and now my maiden name is my middle name per social security and driver's license. When applying for a passport, I put my first name maiden name, and new last name. But issued passport shows my old middle name with my new last name; therefore no maiden name and does not match other documents. Do I need to change this? Thanks.

Answer: It is ideal but not necessary for your passport and IDs to exactly match. Therefore, you may elect to change the name on your passport but you do not have to. You just need to make sure that when booking tickets, use the name that's on your passport.

Will I be okay? Ideally, I would have everything match. If I am leaving 2 months from now, should I try to get my passport changed to include my middle name?

Routine service takes up to 14 weeks and expedited service 10 weeks. It is unlikely that you will receive the passport in time if you apply for a correction before your trip.

Middle Initial
by: Anonymous

Question: After reading the comments/answers in this thread. Middle initials are OK for passports, even REAL ID (it seems are OK -in some states - with it as long as they match the passport). My question is if one wants to revert back to full spelling of their middle name (as it is in their birth certificate for example). What is the process? Most commercial documents, if not all, already abbreviate middle names even if the full name is in their systems.

Answer: If you would like to have your middle name fully spelled on your passport, you need to submit a new application as if it's your first.

Question: I want to renew my passport. My passport about to expire has my middle name on it. I want to keep it out of my new passport. My birth certificate doesn’t include my middle name. Any issues, or should I include my birth certificate with my renewal form?

Complete Form DS-11 without the middle name and submit your birth certificate and ID without the middle name as well.

Renewed passport initialized my middle name
by: Joseph

Question: My passport expired in May, I renewed it two months ago and finally got my new passports. However, even though completing the DS82 form accurately, human error somehow combined my "second first name" and my "middle name" together as initials.

In other IDs, I have used my second first name and middle as initials but not on driver licenses or passports.

Example: If my legal name is as follows.
First: John Michael
Middle: Richard
Last: Kimble

My old passport would read correctly as:
Last: Kimble
Given Name: John Michael Richard

Instead, my new passport came back as:
Last: Kimble
Given Name: John MR

Note the "MR" isn't even spaced out. I probably would have let it go if it was Kimble, John M R

Am I worrying about my issue too much, is it ok to just have JOHN MR KIMBLE in my passport when it should have really read JOHN MICHAEL RICHARD KIMBLE?

Any advice is appreciated before I have to wait another two months for my passport.

Thanks!

Answer: Travel-wise, it should not be an issue as long as you book your tickets in the name that appears in your passport. However, some people have reported having issues with obtaining Real IDs if there is a mismatch between documents. You may want to put this under consideration if you don't have Real ID yet.

No middle name on birth certificate
by: Anonymous

Question: My birth certificate does not have a middle name listed. When my parents wanted to add a middle name shortly after I was born, their attorney told them they did not need to legally change it, but if they just started using it on all paperwork, school documents, etc it would become my legal name. Now the NY DMV is giving me a hard time about it. My new license is a standard license because I could not get an enhanced license with my middle name without a legal name change. The standard license, my SS card, all tax documents, etc all have my middle name included. Will I be able to obtain a passport with my middle name included since all but my birth certificate already includes it? Or will I need a legal name change.

Answer: You may have your middle name on your passport as long as it is supported by the identification documents you submit.

Answer: Leniency should be given when issues regarding middle names/initials arise. You may not need to change your passport. Please contact the airline to confirm.

Question: My current (expired) passport does not include my middle name or initial. I booked overseas plane tickets and included my middle initial on the reservation. I am renewing my passport and want to add my middle initial so that my passport matches my ticket. Prior FAQs say to include a copy of my birth certificate or social security card when submitting my application. However, I changed my middle name when I got married so my birth certificate does not list my current middle name and my social security card has my full middle name listed (instead of just an initial). If include my social security card with a passport application that lists my middle initial and not the fully spelled out middle name, will the application be rejected?

Answer: The SS card should suffice. The application will most likely be accepted but be prepared to answer questions should they arise.

Drivers license not matching Passport
by: Anonymous

Question: I understand that to rent a car in Europe, you need to present your passport. My passport has always been in my maiden name but because I use my drivers license in the US as ID for my credit cards & because of my children, I use my married name. Should I legally change my name adding my maiden name as a middle name or would it be best to Hyphenate my maiden name with my married name? Would this simplify having to change all my things to one name or the other? What is your opinion?

Answer: It would be best to have your passport bear your current legal name.

Question: So even though my birth certificate is correct and my middle name on my license is misspelled by one letter but my application matches my birth certificate, I will have issues? If so, what supporting documents will be needed?

Answer: There is no telling for sure if you will have issues. The final say on the name will be of the passport agent who processes your application. If they need more documentation, they will most likely ask for documents at least 5 years or older bearing the name you enter in the application form.

Question: The last letter of my middle name is spelled wrong on my license. My birth certificate says Laquah and my license said Laquan. Will this stop me from getting a passport? I put Laquah on my application as well.

Answer: It is best to have your license corrected first. While there is a chance that your passport will be issued in Laquah because it matches your proof of citizenship, questions might arise. This is why we recommend having your identification documents match before applying for a passport.

Middle name

Question: When I married the 1st time and changed my last name on my driver's license the license bureau switched my maiden name as my middle name not my birth middle name. It is correct on my first and last name but what shows on my driver's license middle name and now my passport shows two different names. I have always flew listing it the way it shows on my driver's license my maiden name as my middle name and not my birth middle name. I had to get a passport for the first time this year since we are flying out of the country next year and the passport office has my birth middle name on my passport. That is what was on my birth certificate. The airlines all have my middle name as what is on my driver's license, my maiden name. So will I need to contact the airlines we will use before I make my flight arrangements and change my middle name to reflect what is on my passport. I have a lot of times been prechecked approved. I have only used my driver's license when I have flown and I have not had a problem. Going out of the country next year I am afraid I will encounter a problem. What would you suggest?

Answer: Please book your tickets in the name that appears in your passport.

Question: I've always used my BAPTISMAL name as my middle name on all legal documents though it is not on my birth certificate, never a problem. Didn't realize it was not my real middle name until recently when I had a problem at DMV in NY, couldn't get an enhanced license. Will I have the same problem getting a passport?

Answer: You most likely will if you enter the middle name in the application. But if can resolve the issue with the DMV and your license will have your middle name on it, your passport should be issued in that name.

Renew or Apply for New Passport
by: Anonymous

Question: My dad accidentally booked a flight for me under my married last name. That isn’t a problem as I am changing my last name. However, for the tickets he did not put my middle name. I currently have my middle name on my passport and it is on my marriage license.

Can I renew my passport with a name change supported by a marriage license that has my middle name, but not fill my middle name on the application? Or will it be denied?

Or is it better to apply for a new passport under my new name and not include my middle name on the application? My husband never put his middle name on his passport even though his supporting documents had it.

I’m not sure if a renewal and name change will let me drop my middle name on the passport.

Answer: You can apply for a renewal with name change. But you need to submit your marriage certificate, not the marriage license.

Question: Hi. I noticed that my passport book did not include my middle name even though on the paperwork I submitted, it included the middle name. Before I got married last year, I did not have a middle name. My marriage license does not have the middle name that I gave myself but my SSN card and driver's license do.

How can I fix it so that my passport matches my driver's license and SS card? My birth certificate does not have a middle name either. Do I even need to include a middle name on my passport as long as I fill out my airline paperwork without a middle name?

I legally added a last name to my name. So on the legal court order it showed my first middle and last name, and on my social it is my first middle and last name but it’s so long that for my state ID, I just wanted it to show my first name and last name so it just shows first and last.

Answer: It will not be rejected. The passport will be issued in the name you enter in the application form as long as it is supported by the ID that you present.

Middle Name Change Not Reflected on Passport
by: Liz

Question: When I got married, I changed my last name to my husband's name. At the social security office, I was able to change my middle name to my maiden name.

When I applied to change my name for my passport, they sent back my passport without the middle name change.

I've been going back and forth with them, and they said I'd have to prove that I've been using "Atkins" as a middle name instead of "Ann" for five consecutive years. I've only been married six months.

Will I run into problems when traveling in April if my middle name is not the same on my passport as it is on my driver's license and social security card?

Answer: You should not have any problems as long as you book flights in the same name that is in your passport. Rarely is it necessary to show your driver's license and/or SS card together with your passport when traveling internationally.

If you want to change the name in your passport, you can petition for a court order name change. Once the court order has been approved, you can send it to change the name in your passport. There is no fee if you submit the application within 12 months of your passport's date of issuance.
